• Annual Health History Update: Completed by parents/guardians for each student at the start of the school year.
• Parental Consent for Over the Counter Medication: Completed by parents/guardians for students in middle school or high to allow the school nurse to administer certain over the counter medications.

Forms for Doctors to Complete

• Concussion - Doctor's Reentry Note: Completed by a physician when a student returns to school after having a concussion.
• Doctor's Allergy Action Plan: Completed by a physician to detail the medications required for students with a food or insect allergy.
• Massachusetts Asthma Action Plan: Completed by a physician to detail the action plan for a student with asthma.
